The CDC has taken steps to simplify COVID-19 vaccine recommendations and is allowing more flexibility for people at higher risk who want the option of added protection. The CDC's Advisory Committee on Immunization Practices (ACIP) now recommends an additional bivalent vaccine dose for adults ages 65 years and older and those who are immunocompromised.

COVID-19 SMART Health Card
COVID-19 SMART HEALTH CARD
Request, view and print your vaccination records and your COVID-19 SMART Health Card from the official Massachusetts Immunization Information System (MIIS).
Please allow up to 24 hours for a response.
If you need to request records for multiple dependents (e.g., children), you'll need to submit a new request for each.
The portal will provide you with a digital copy of the record of vaccines which have been reported to MIIS. This system only includes records that your healthcare providers reported to Massachusetts. If you need a record of vaccines you received from the federal government or in another state, you'll need to contact whoever administered those vaccines.
Submit your request to the State's Vaccine Records page.
My Vax Records is just one way residents can obtain their COVID vaccination record. Pharmacies that administered the COVID vaccine and many health care providers also are making SMART Health Cards available, or are providing additional options. Learn more about Smart Cards and other vaccine options.
