Community Health Equity Survey Promotion Toolkit

The Community Health Equity Survey (CHES), previously known as the COVID-19 Community Impact Survey (CCIS), is now live! It aims to improve health conditions in impacted communities by gathering data on housing, mental health, safety, and other health drivers. Collaboratively developed with community members and partners across Massachusetts, the survey will inform funding allocation, program improvement, and policy development to address health inequities. To spread the word, you can share the survey link, promote it on social media, post flyers, and encourage participation at community events. The survey is open until early Fall 2023 for individuals aged 14 and above, available in multiple languages. 

Data will be accessible on the Massachusetts Department of Public Health (MA DPH) website after the survey ends, empowering communities to take action for health equity.

The Brookline Department of Public Health and Human Services is integrating the Community Health Equity Survey into our Community Health Assessment and Improvement Planning process. This strategy aims to streamline efforts and avoid redundant surveys. Your support is appreciated, and dissemination materials are provided below.
