Please upload a small sketch diagram of your property. It should include the following:
Diagram may be drawn by hand or computer generated - it only needs to be a sketch, but please be as accurate as possible. Three example property diagrams are included below - please note you only need to submit one diagram.
If you are experiencing technical difficulties or cannot submit a diagram at this time, you may submit your sketch separately. Just complete and submit the remainder of this form, and then you may send the diagram either via email to firstname.lastname@example.org or mail it to our office at Tom Brady, Tree Warden, Brookline Parks and Open Space Division, 333 Washington Street, Brookline MA 02445. Be sure to indicate the property address so we can match it to your application. For additional assistance, please contact the Parks and Open Space Division at email@example.com.
Example 1: Hand Drawn Diagram If you feel comfortable drawing the layout of your property, this is a great option! Once you have drawn out all the relevent information, scan in (or take a photo of) your diagram and upload it to this form.
Example 2: Annotated Photo Take a photo of your property (be sure to include overhead electric lines, driveways, street trees, etc.) and annotate where you would like the front yard tree(s) to be planted. You can either print out the photo and annotate it by hand, or annotate it electronically. Once completed, please upload your picture to this form.
Example 3: Assesor's Abutters Program Visit https://gisweb.brooklinema.gov/Abutters/ and search for your house number and street. A map of your property will automatically be generated. In the top right hand corner of your screen, click "Print Map." You can then either print the document and annotate it by hand, or save it as a PDF and annotate it electronically. (One good trick for saving the file as a PDF is to click "Print" and choose "Adobe PDF" as your printer. You will be prompted to give the file a name and it will save in PDF format, which you can save and then digitally anotate.) Once completed, please upload your picture to this form.
A representative list of species that are often available can be found halfway down the page (under the "Example Tree Species" tab) at www.brooklinema.gov/frontyardtree. Please indicate your preference(s) in the answer box below. Not sure what kind of tree(s) would work best for your site? No problem- simply leave this answer box blank. The Tree Planting Committee and Tree Warden would be happy to offer suggestions..
